I hereby assume all risks of, and responsibilities for, accidents and/or damage to myself, my property, or to others resulting form the actions of my dog(s) or myself. I expressly agree that Malinda Lytle, or no other person, or persons shall be held liable individually or collectively, under any circumstance, for injury and damage to my person or for loss or injury to my property, whether due to uncontrolled dogs or negligence of any member or instructor of said group or any other cause or causes. In consideration of the acceptance of entry while exercising or training a dog, I (we) agree to hold Malinda Lytle, the owner or lessor of the premises and any employees or agents of the aforementioned parties form any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of my dog(s) while in or upon exercising or training a dog at the premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim: and I(we) further agree to hold the aforementioned parties harmless from any claim for loss of this dog by disappearance, theft, death or damage or injury, be caused or alleged to be caused by the negligence of any of the parties aforementioned, or by the negligence of any person, or any other cause or causes.

I (we) hereby assume the sole responsibility for agree to indemnify and save all the aforementioned parties harmless from any and all loss and expenses (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death at anytime resulting therefrom, sustained by any person including myself (ourselves) on or account of damage to property arising, out of or inconsequence of my (our) participation while exercising or training a dog, howsoever such injuries, death or damage to property may be caused, and whether or not the same may have been caused or may have been alleged to have been caused by negligence of the aforementioned parties or any of their employees or agents, or any other persons.

I will bring only healthy animals, current on all vaccinations or titers, and promise to keep my dog(s) under my control at all times. I will not allow my dog(s) to disturb other exhibitors or patrons or the property. At no time will I abuse my dog(s) or engage in unsportsmanlike conduct. This applies to all activities regarding Malinda Lytle now and all future visits.

Further, I have read and accepted the terms of this form.
